Job Title: Patient Navigator Ambulatory | Oncology | Bemidji

Job Schedule: Full Time
Hours Per Shift: 8 hr
Coordinate, negotiate, procure services for, and manage the care of, complex patients to facilitate achievement of quality and cost-effective patient outcomes. Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable.
Full-time, 80 hours every pay period, day shift.
Requires current licensure in area of expertise in the state/states of practice. Certified Case Manager Certification (CCM) or certification in navigation preferred. Bachelor's degree in Social Work, Nursing, or Counseling is required, Master's degree preferred. Three years of experience and three years of management/leadership experience desired. Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable.
